INDEPENDENT LIVING PROGRAM

for Teens and Youth Adults

Providing youth and young adults from Josephine and Jackson Counties how have been in foster care help to transition into adulthood. The Independent Living Program (ILP) approach to build relationships with the youth while teaching important life skills that will prepare them for living on their own with their own community supports. Hearts can currently serve up to 60 youth in Josephine and Jackson County through funding by the Oregon Department of Human Services.

SPARK: Ages 14-15

The SPARK program to support young people who experience foster care as they build capacity across five distinct SPARK domains to promote self-determination and support positive outcomes.
Story of Self / Personal Narrative
Understand how to tell parts of your story to protect self and others (strategic sharing), to connect with others and influence change.
Purpose
See self as able to serve others, rather than solely as a recipient of services. Knows how lived experience can contribute to others.
Attachment & Connectedness
Willing to put your trust in other people, as appropriate. Skills to establish healthy boundaries, permanence and social capital.
Regulation
Understand how past trauma might impact behaviors and responses to situations, and build skills to self regulate in healthy ways.
Knowledge of System
Knows how to navigate the system and understands how system may impact you. Knowledge and skills to acess resources.

Monthly Connection. Connect for peer support, community and mentors.

ILP Plus Ages 16-21

Weekly Connection. This is for young people who need more than a monthly connection. They may have higher needs or need some extra help.

It is not about diagnosing or counseling - it is about skill building

Foster kids go into careers that they are exposed to, Social Work &  Law enforcement. We like to bring in other professionals to expose them to other career possibilities.
