What is HRSN?

HRSN stands for Health-Related Social Needs, a Medicaid benefit through CareOregon.

It helps address non-medical needs that impact a person’s health especially housing and utilities.

The goal is housing stability and prevention of homelessness, not housing placement.

What We Help With

Support may include:

  • Rent assistance (up to 6 months, if eligible)

    • Utility assistance* (electric, gas, phone, internet, etc.)

    • Storage costs* (up to 6 months, if eligible)

  • Tenancy support (helping maintaining current housing)

  • Connection to related supports (nutrition, climate-related needs, and home safety modifications)

* Must be receiving Rent Assistance to qualify for potential Utility and Storage Cost helps.

STEP 1

Initial HRSN Screener

Health-Related Social Needs (HRSN) is a Medicaid benefit through the Oregon Health Plan (OHP) that may help eligible members with housing stability and utility support. HRSN housing benefits may include but are not limited to:

  • Rent support
    • Utility payments
    • Storage assistance
  • Housing navigation and tenancy supports 
  • Home Modifications for a Medical Condition

Please answer these basic eligibility questions to see if you may qualify for HRSN support.

Basic Eligibility Questions

  1. Are you currently enrolled in the Oregon Health Plan (OHP)
  2. Are you currently renting and have a lease or  written rental agreement?
  3. Are you at risk of losing your current housing?
  4. Do you currently have any of the following?
    •  Medical Condition
    • Mental health Condition 
    • Disability
    • Child under 6 living in the home
    • Are 65 or older
  5. Have you experienced any of the following within the past 12 months?
    • Release from incarceration
    • Discharge from a mental health or treatment facility
    • Foster care/child welfare involvement
    • Transitioning from Medicaid to Medicare
    • Domestic violence or unsafe housing
    • Pregnancy or Postpartum?

Additional documentation may be required. Eligibility is based on OHP enrollment, housing risk, qualifying life transitions, health-related needs, and required documentation.
